Highland Park Rehabilitation and Nursing Center has a social worker on staff to help the resident and their family members adjust to placement. Working with the psychosocial needs that may arise during the course of the individual’s stay at the facility, the social worker is not only a source of emotional support, he or she also assists with any necessary adjustments to facility living, mood, financial, and individualized concerns or family requests.

Highland Park Rehabilitation Social Services

Our social worker regularly meets with families and residents to discuss, evaluate and problem-solve so that the resident can focus on rehabilitation and medical needs with minimal stress and distraction. Residents and families are encouraged to meet with the team members as necessary to resolve any special concerns. These appointments are coordinated through the Social Services office at a mutually convenient time.

The residents of Highland Park have a “Resident Council” meeting once per month which gives the residents an opportunity to discuss issues pertinent to the quality of their lives. The Social Worker and the Activities Director assist the Resident Council Representatives to maintain an active, involved and united voice that speaks for all our residents. These recommendations are communicated to the Administration for continued quality improvement and services.
